Self assessment medical form and medical assessment details

We want diving to be as safe as possible and as such we work with the UK Diving Medical Committee (UKDMC) using their national network of medical referees for any medical assessments which may be required.

 

Self Assessment

For most divers, completing the annual self assessment medical declaration form is easy and no follow up is required. To download the latest version of medical forms visit http://www.ukdmc.org/downloads/. Upon joining the SAA all members need to complete one of these forms and any medical concerns are then to be followed up by a dive medicine expert from within the UKDMC network of medical referees. This self assessment medical form needs to be completed annual as one of the requirements of membership of the SAA.

Follow Up

Arranging a consultation with a dive medicine expert to discuss an existing medical condition is easy. The UKDMC’s medical referee locator is the best way to find your local expert so you can continue your scuba diving journey, http://www.ukdmc.org/medical-referees/. Here at the SAA we recommend any medical concerns you have are followed up by one of the UKDMC medical referees.
